fa بررسی واریانت (rs 5219) از ژن KCNJ11 در بیماران مبتلا به دیابت بعد از پیوند کبد Investigation of KCNJ11 Gene Variant (rs 5219) in Patients with Diabetes after Liver Transplantation ژنتيك پزشكي Medical Genetics پژوهشي Research زمینه و هدف: ابتلا به دیابت در گیرندگان پیوند کبد یکی از مشکلات رایج در این بیماران بوده که دارای تأثیر منفی در موفقیت پیوند و حیات بیمار می‌باشد. دیابت پس از پیوند (PTDM) از نظر پاتوفیزیولوژی، بالینی و ژنتیکی با دیابت نوع 2 شباهت‌های زیادی دارد. یکی از ژنوتیپ‌های دخیل در ایجاد PTDM، 5219rs در ژن KCNJ11 می‌باشد که در عملکرد سلول‌های بتا دخیل بوده و اثر تنظیم کننده بر کانال پتاسیمی حساس به ATP دارد. هدف از این مطالعه، بررسی ارتباط میان این پلی مورفیسم و دیابت پس از پیوند کبد در یک جمعیت ایرانی می‌باشد. مواد و روش‌ها: این مطالعه، یک مطالعه مورد شاهدی می‌باشد که بر روی 120 بیمار دریافت کننده پیوند کبد (60 بیمار مبتلا و 60 بیمار غیر مبتلا به دیابت پس از پیوند) انجام گرفته است. نمونه گلبول‌های سفید از بانک سلولی مرکز تحقیقات پیوند اعضا بیمارستان نمازی شیراز از بیمارانی که در سال‌های 1390-1386 پیوند کبد دریافت کرده‌اند انتخاب شده است. فراوانی ژنوتیپ‌های مختلف در این افراد با روش PCR-RFLP تعیین گردیده است. برای بیان نتایج از شاخص‌های پراکندگی و تست‌های آماری پارامتری استفاده شد.<o:p /> نتایج: آنالیز آماری نشان می‌دهد که اختلاف معناداری بین فراوانی آللی و ژنوتیپی در دو گروه بیمار و کنترل وجود دارد و ژنوتیپ KK با (001/0 =p) و (6/7-60/1)CI%95,19/1=OR، ریسک ابتلا به PTDM را افزایش می‌دهد.<o:p /> نتیجه گیری: در این مطالعه بین بروز دیابت پس از پیوند با جنس، سن، دوز پردینوزولون، پلی مورفیسم KCNJ11 و ژنوتیپ KK ارتباط وجود داشت.<o:p /> Background & Objective: The incidence of diabetes in liver transplant recipients is one of the common problems among these patients witha negative effect on the success of the transplant and the patient's life. Post Transplant Diabetes Mellitus (PTDM) has many similarities with type 2 diabetes from pathophysiplogical, clinical, and genetic viewpoints... One of the genotypes involved in PTDM is KCNJ11 (rs 5219) that has role in beta-cell function and has regulator effect on ATP sensitive K channels. The purpose of this study is to review the relation between this polymorphism and diabetes after liver transplantation in an Iranian population.<o:p />Materials & Methods: This is a case-control study on 120 patients receiving liver transplant, including 60 patients with PTDM and 60 patients without PTDM. White blood cell samples were selected from the cell bank of organ transplantation research centre in Namazi Hospital from patients who had undergone liver transplantation during the years 2008 and2012. Frequency of genotypes in this population was determined by PCR-RFLP method. To express the results we used distribution indices and parametric statistical tests.<o:p />Results: Statistical analysis shows that there is a significant difference between the genotype and the allele frequencies of the two groups of case and control.. KK genotype (P=0.001 95%CI (1.60-7.6) OR=1.19 increases the risk of PTDM.<o:p /> Conclusion: Our study shows that there is a relation between the incidence of PTDM and sex, age, prednisolone dosage, KCNJ11 polymorphism, and KK genotype.
